    Provide person-centred support to enable the client to remain safe, comfortable and as independent as possible in her own home. Support includes: • Assist with personal care, washing and bathing as required, while maintaining dignity and privacy., • Support the client with dressing and getting ready for the day., • Support and encourage safe mobility around the home., • Encourage the client to maintain independence and complete tasks she is able to do safely herself., • Prepare breakfast, light meals, snacks and drinks according to the client's preferences., • Encourage adequate food and fluid intake and report any concerns., • Provide support with toileting and continence needs where required, maintaining dignity and good hygiene., • Provide companionship, reassurance and emotional support., • Encourage the client to follow mobility/physiotherapy exercises already recommended by relevant professionals, where these form part of her agreed care plan., • Observe for changes in mobility, pain, skin condition, bowel pattern, appetite, hydration or general wellbeing and report concerns promptly., • Maintain a safe environment and reduce falls risks, particularly when the client is mobilising., • Record all care and relevant observations accurately after each visit., • Report any deterioration, incidents or concerns in accordance with OPCS escalation procedures. Skills Required • Personal care and dignity, • Bathing and washing support, • Continence and toileting support, • Safe mobility assistance, • Moving and handling awareness, • Falls prevention and risk awareness, • Meal and drink preparation, • Nutrition and hydration monitoring, • Observation and escalation of changes in condition, • Skin integrity awareness, • Supporting an older person with reduced mobility, • Promoting independence, • Person-centred communication, • Companionship and reassurance, • Accurate care documentation, • Infection prevention and control, • Medication awareness and safe working within the care plan, • Ability to recognise when concerns require escalation to family, manager or healthcare professionals Equipment / Aids Equipment currently identified or requiring confirmation: • Walking/mobility aid used by the client, • Stair support/handrails within the home, • Appropriate footwear for safe mobility, • PPE for personal care as required Important The successful applicant must: • Live in or very close to Sydenham (SE26) or be able to travel reliably to the area for scheduled calls., • Be available for morning, lunchtime and evening visits, including weekends., • Be reliable and able to commit consistently to agreed visits., • Have experience providing personal care, including washing, bathing and dressing., • Be confident supporting a person with reduced mobility and promoting safe movement around the home., • Understand how to encourage independence rather than taking over tasks unnecessarily., • Be able to prepare simple, nutritious meals and drinks and confidently encourage good nutrition and hydration., • Be comfortable providing medication prompts/support in accordance with the client's care plan and medication records., • Be able to observe and accurately record food and fluid intake and report concerns appropriately., • Have good communication and observation skills and recognise changes in a client's wellbeing that require escalation., • Understand dignity, privacy, choice and person-centred care., • Have good written and spoken English for communication and accurate care records., • Follow infection prevention, safeguarding, moving and handling and medication procedures., • Complete required care records accurately and promptly after each visit., • Be willing to complete OPCS recruitment, training and competency requirements before working independently. Desirable • Previous domiciliary/home-care experience., • Experience supporting older adults with reduced mobility., • Experience supporting clients with nutrition and hydration needs., • Relevant care qualifications or training., • Driver with access to a vehicle, although applicants who live locally and can reliably attend all required visits will also be considered.
