We undertake Care Act assessments and support planning, which can include Mental Capacity Assessments and Best Interest decisions, often following the Discharge to Assess (D2A) pathway. The complexity of our work varies greatly, supporting all adults of the hospital who are Surrey residents with a need for care and support.
