Job Description Position: Director of Patient Services (RN) Program: Briarwoods Assisted Living Program (ALP) Employment Type: Full-Time (weekdays, with on-call availability as needed) Salary: $85,000+ annually, commensurate with experience About Briarwoods Briarwoods is a warm, resident-centered Assisted Living Program (ALP) providing housing, personal care, and home health services to adults who need daily support in a homelike setting. Our interdisciplinary team is dedicated to helping residents age in place with dignity, safety, and as much independence as possible. Position Summary Briarwoods is seeking a compassionate, experienced Registered Nurse to serve as Director of Patient Services (DPS) for our Assisted Living Program. The DPS is the registered nurse responsible for the direction and supervision of patient care services and other health care activities of the program, ensuring high-quality, person-centered care in full compliance with all applicable state regulations. This is a hands-on leadership role that blends clinical oversight, staff development, care coordination, and regulatory compliance. Key Responsibilities • Direct and supervise all patient care services and health care activities of the ALP, including nursing, personal care, and home health aide services, • Oversee resident assessments, reassessments, and individualized service/care plans, including admissions, level-of-care determinations (UAS-NY), and coordination of services, • Supervise, mentor, and evaluate nursing staff, home health aides, and personal care aides; oversee orientation, in-service education, and annual competency requirements, • Ensure compliance with all applicable state regulations and agency policies and procedures; prepare for and participate in NYS DOH surveys and audits, • Develop, implement, and update clinical policies, procedures, and quality assurance/performance improvement (QAPI) activities, • Coordinate care with physicians, pharmacies, hospice, managed care plans, families, and community providers, • Oversee medication management systems, infection control practices, and incident reporting/investigation, • Monitor resident health status and respond to changes in condition; ensure timely communication with physicians and responsible parties, • Participate in case management, interdisciplinary care meetings, and discharge/transfer planning, • Manage clinical documentation and ensure records meet regulatory and billing requirements (including Medicaid ALP requirements), • Share on-call responsibility for clinical issues as needed Qualifications Required: • Current, unencumbered license and registration as a Registered Nurse (RN) in New York State, • Minimum 2 years of clinical nursing experience, preferably in home care, assisted living, long-term care, or geriatrics, • At least 1 year of supervisory, administrative, or leadership experience in a health care setting, • Working knowledge of NYS home care regulations; familiarity with assisted living and adult home regulations strongly preferred, • Strong leadership, communication, organizational, and problem-solving skills, • Proficiency with electronic health records and clinical documentation Preferred: • Bachelor of Science in Nursing (BSN), • Experience with UAS-NY assessments and Medicaid ALP documentation and billing requirements, • Prior experience as a Director of Patient Services, Assistant DPS, or Nursing Supervisor in home care or assisted living, • Experience with survey readiness and quality improvement programs What We Offer • Competitive salary starting at $85,000, commensurate with experience, • Comprehensive benefits package including health insurance, paid time off, and holidays, • Retirement savings plan, • Supportive leadership and a collaborative, mission-driven team, • Meaningful work improving the daily lives of our residents How to Apply Please submit your resume and a brief cover letter to Mindy Gray, Human Resources Manager; mgray@briarwoodmanor.com.
