Job Description Director of Population Health Location: Syracuse, New York ABOUT THE OPPORTUNITY Our client, Crouse Health, is seeking an experienced Director of Population Health to provide strategic leadership across population health, value-based care, clinical integration, utilization management, and quality improvement initiatives. This role will be responsible for developing and implementing programs that improve clinical outcomes, enhance the patient and provider experience, and support the delivery of high-quality, cost-effective care across Crouse Health’s member populations. This is a highly collaborative leadership role with the opportunity to influence population health strategy across the organization, strengthen provider and payer relationships, and develop scalable programs that support improved health outcomes and long-term value-based care performance. The Director will also oversee departmental operations and staff while serving as a key resource to senior leadership, committees, and clinical stakeholders. CORE RESPONSIBILITIES • Develop and maintain effective utilization management and quality management systems supporting credentialing, utilization review, cost management, member performance feedback, communication, and related administrative functions., • Identify key needs across Crouse Health's member populations and develop scalable, data-driven programs designed to improve outcomes and reduce the total cost of care., • Provide strategic guidance to leadership and support short-, mid-, and long-term planning aligned with organizational goals., • Promote collaboration and alignment among physicians, practices, and health system partners., • Design and implement evidence-based protocols to streamline chronic disease management., • Translate utilization, cost, and quality data into actionable insights for physicians and clinical stakeholders., • Support payer partnerships and serve as a primary point of contact., • Facilitate process improvement activities supporting clinical integration initiatives., • Assist leadership and boards/committees in identifying and promoting methods to improve the quality of healthcare provided by participating members., • Develop and maintain effective member grievance processes that ensure timely response and resolution., • Maintain current knowledge of healthcare industry trends, market conditions, regulatory requirements, quality mandates, reimbursement requirements, and third-party payer utilization standards., • Ensure compliance with applicable federal and state regulations, regulatory/accrediting guidelines, and professional standards., • Participate in hospital and performance improvement councils and committees as requested., • Develop and maintain department policies, scope of services, quality indicators, and related procedures., • Manage and develop staff, including employee selection, onboarding, coaching, performance management, staffing, work assignments, and goal alignment., • Coordinate resources and budgetary needs to support quality, cost-effective programs and services., • Foster strong relationships with physicians, healthcare providers, community partners, and other stakeholders., • Communicate organizational and industry trends to leadership and recommend modifications in strategy or direction as needed., • Perform other duties as assigned by Senior Leadership. REQUIREMENTS • Bachelor's degree in a related field required., • Master's degree (MS, MPH, MBA) or other licensed clinical degree (LPN, RN, NP) preferred., • Minimum of five years of experience in a related field., • Prior experience in population health, value-based care performance, clinical integration, utilization and care management, physician engagement, and working directly with payer partners., • Experience managing healthcare quality improvement, performance improvement, and/or practice transformation projects., • Proven business acumen and analytical skills, including an understanding of clinical data management and analysis., • Strong written and verbal communication skills with excellent facilitation and interpersonal abilities., • Excellent organizational and problem-solving skills., • Current knowledge of governing regulations, third-party payer utilization, quality mandates, reimbursement requirements, and standards associated with Care Management and Utilization Management., • Demonstrated ability to effectively interact with physicians, patients, healthcare providers, and team members.
