Job Description Job Title: Lead Case Manager Job Classification: Exempt Reports to: Programs Director Department: Rally Veterans Housing Program Summary/Objective: The Lead Case provides overall leadership and day-to-day oversight of the Rally Veterans Housing Program, ensuring high-quality case management, resident engagement, and compliance with VA Grant and Per Diem (GPD) requirements. The Lead Case Manager directly provides case management services while coordinating program operations, supporting Program Safety Monitors, volunteers, and fostering a safe, welcoming, and recovery-oriented environment. Working from a Housing First, trauma-informed, and harm reduction philosophy, the Lead Case Manager collaborates closely with VA partners, healthcare providers, PPCS Campus Safety and Operations teams, and community agencies to ensure Veterans receive coordinated services that promote housing stability, self-sufficiency, and long-term success. This position manages essential staff that are required to work during holidays or inclement weather, and Lead Case Manager will be expected to respond to after-hours’ crises’ needs. Essential Functions: • Provide day-to-day leadership and oversight of the Veterans Housing Program., • Maintain a caseload of 15 Veteran residents and provide individualized case management services in accordance with VA Grant and Per Diem (GPD) requirements., • Complete program intakes, assessments, individualized service plans, progress notes, case coordination notes, incident reports, discharge summaries, and post-discharge planning within required timelines., • Work collaboratively with Veterans to establish and achieve goals related to permanent housing, income, employment, healthcare, recovery, and long-term stability., • Coordinate admissions, discharges, and ongoing communication with the VA Health Care for Homeless Veterans (HCHV) Grant and Per Diem Liaison and other referral partners., • Serve as the primary program contact with VA partners and coordinate services with healthcare providers, behavioral health providers, housing agencies, and community organizations., • Provide day-to-day directions, coaching, and support to the Veterans Housing Program Safety Monitors to ensure consistent resident engagement, communication, documentation, and program operations., • Meet regularly with Program Safety Monitors to review resident progress, safety concerns, behavioral issues, and follow-up needs while promoting a consistent, resident-centered approach across all shifts., • Review incident reports, shift communications, and resident documentation to ensure timely follow-up and coordinated service delivery., • Work collaboratively with the PPCS Programs team and Campus Safety and Operations teams to maintain a safe, supportive, and low-barrier living environment while balancing resident needs with Campus safety and operational priorities., • Connect Veterans to housing resources, VA benefits, public benefits, employment services, healthcare, transportation, and other community resources that support successful housing outcomes., • Assist Veterans with obtaining identification, income, employment, permanent housing, and other resources necessary for successful community living., • Utilize Housing First, trauma-informed care, motivational interviewing, and harm reduction principles to build trust, reduce barriers to engagement, and support positive outcomes., • Coordinate and facilitate life skills, financial literacy, employment readiness, wellness, and other educational or supportive group activities., • Support residents with daily living skills, budgeting, medication management, housing readiness, and successful transition to permanent housing., • Maintain accurate client files, HMIS and other required data systems, census information, and program documentation in accordance with agency, VA, CARF, and funding requirements., • Complete required reports, outcome measures, and other documentation within established timelines., • Respond appropriately to resident concerns, behavioral issues, and crisis situations using sound judgment, de-escalation techniques, and trauma-informed practices., • Build and maintain strong relationships with community providers and resource partners to expand services available to Veterans., • Participate in interdisciplinary case conferences, staff meetings, supervision, training, and quality improvement activities., • Perform other duties as assigned. Housing Stability • Assist participants with understanding lease obligations, tenant responsibilities, and applicable program expectations., • Identify issues that may jeopardize housing stability and intervene as early as possible., • Work collaboratively with participants, landlords, property management, and program leadership to address housing-related concerns., • Assist participants with developing strategies to prevent eviction and resolve lease violations when possible., • Support participants in maintaining safe and sanitary living environments., • Coordinate reasonable supportive interventions when health, behavioral health, financial, or other circumstances threaten housing stability., • Assist participants with housing transitions when a move is necessary and permitted under program requirements., • Support participants in pursuing longer-term housing options, rental assistance, or other housing opportunities when appropriate. Service Coordination & Community Resources The Lead Case Manager will connect participants with appropriate community-based services and mainstream resources, which may include: • Medicaid, Medicare, SNAP, SSI, SSDI, and other public benefits., • VA Benefits, • Primary and specialty healthcare., • Mental health and behavioral health services., • Substance use treatment and recovery supports., • Employment and workforce development. Required Qualifications • Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, or a related field. Equivalent education and relevant experience may be considered., • Minimum of two years of experience providing case management or supportive services to individuals experiencing homelessness, behavioral health challenges, substance use disorders, Veterans, or other vulnerable populations., • Experience coordinating programs, leading staff, or providing day-to-day direction within a residential, shelter, or supportive housing setting preferred., • Knowledge of or commitment to Housing First, trauma-informed care, motivational interviewing, harm reduction, and person-centered service delivery., • Ability to build positive working relationships with Veterans who have experienced homelessness and other complex challenges., • Experience coordinating community resources and multidisciplinary services., • Strong leadership, organizational, documentation, and problem-solving skills., • Ability to manage multiple priorities while meeting required documentation and reporting deadlines., • Strong written, verbal, and interpersonal communication skills., • Proficiency with Microsoft Office, HMIS, and electronic case management systems or the ability to learn., • Valid driver's license and ability to travel within the community as needed. Preferred Qualifications • Experience working with Veterans experiencing homelessness., • Knowledge of military culture, VA Grant and Per Diem (GPD) requirements, and VA resources., • Experience working in transitional housing, emergency shelter, permanent supportive housing, or other residential programs., • Familiarity with community housing resources, public benefits, and behavioral health systems., • Experience facilitating life skills, educational, or wellness groups., • CADC, RASAC, LMSW, LCSW, LPC, or other relevant human services credential preferred but not required. Supervisory Responsibility: This position has supervisor responsibilities. Work Schedule: Generally, days, nights, and weekends Work Environment: SRO Environment Physical and Environmental Requirements: Ability to lift and carry at least 10 lbs. Travel: Travel is primarily local during the business day.
