Job Description The Case Manager is responsible for providing comprehensive case management services to individuals receiving substance use disorder (SUD) treatment and related behavioral health services. The Case Manager works collaboratively with clients, counselors, therapists, medical providers, peer support staff, families, community agencies, and other members of the treatment team to identify client needs, develop individualized service plans, coordinate resources, and support clients in achieving treatment and recovery goals. The Case Manager serves as an advocate and resource for clients while helping remove barriers that may interfere with successful treatment, recovery, stability, and community reintegration. The ideal candidate demonstrates strong organizational skills, compassion, professionalism, sound judgment, confidentiality, and the ability to work effectively with individuals experiencing substance use, behavioral health, housing, employment, transportation, financial, legal, and other life challenges. ESSENTIAL DUTIES AND RESPONSIBILITIES • Client Assessment and Case Management, • Conduct case management assessments to identify client strengths, needs, barriers, risks, and available resources., • Obtain information regarding housing, transportation, employment, education, financial stability, legal matters, healthcare, family support, benefits, and other social determinants of health., • Assist in identifying client-centered goals and developing individualized case management plans., • Review client needs regularly and update case management plans as appropriate., • Monitor client progress toward identified goals., • Provide ongoing support, encouragement, advocacy, and referrals., • Maintain regular contact with assigned clients in accordance with program requirements., • Assist clients in identifying realistic short-term and long-term recovery and life goals. Help clients develop skills necessary to achieve greater independence and stability. • Treatment Team Collaboration, • Participate as an active member of the interdisciplinary treatment team., • Communicate appropriately with counselors, therapists, nurses, medical providers, peer support staff, program leadership, and other treatment professionals., • Provide relevant case management updates during treatment team meetings., • Coordinate services to prevent duplication, gaps, or delays in care., • Communicate significant changes in client circumstances to appropriate clinical or supervisory personnel., • Support the development and implementation of individualized treatment and discharge plans. . Resource Coordination and Referrals • Assist clients with accessing appropriate community resources, including:, • Housing and emergency shelter services., • Transportation assistance., • Food assistance and nutritional resources., • Employment and vocational programs., • Educational and workforce development opportunities., • Government benefits and entitlement programs., • Health insurance and healthcare services., • Primary care and behavioral healthcare., • Dental and vision services., • Recovery support services., • Community support groups., • Childcare and family support services. Legal resources. • Identification and vital-record services., • Financial assistance programs., • Social service agencies., • Other community resources necessary to support recovery and stability. The Case Manager is responsible for following up on referrals and documenting whether services were successfully accessed. • Recovery and Discharge Planning, • Begin discharge planning early in the client's treatment episode., • Assist clients in identifying appropriate continuing-care and recovery resources., • Coordinate referrals to outpatient treatment, residential programs, recovery housing, community services, and other appropriate levels of care., • Assist clients with establishing appointments prior to discharge when appropriate., • Help clients identify transportation and other logistical needs related to continuing care., • Provide clients with information regarding recovery supports and community resources., • Participate in aftercare planning and coordination., • Follow up with clients after discharge when permitted and required by the program., • Document discharge planning activities and referrals. . Client Advocacy • Advocate for clients' appropriate access to treatment, healthcare, social services, and community resources., • Assist clients in navigating complex systems and completing required applications and paperwork. Help clients communicate their needs to appropriate agencies and service providers. Promote client independence rather than creating unnecessary dependence on case management services. Respect client choices, dignity, culture, privacy, and personal preferences. Identify barriers that may prevent clients from successfully participating in treatment or continuing care. 1. Documentation and Recordkeeping The Case Manager must maintain accurate, timely, objective, and complete documentation. Documentation may include: Case management assessments. Case management notes. Individual service plans. Progress notes. Referral documentation. Resource coordination records. Treatment team documentation. Discharge planning documentation. Follow-up documentation. Client correspondence. Community resource information. Other documentation required by the program, payer, accreditation body, or applicable regulations. All documentation must be completed within required timeframes and must accurately reflect services provided. 1. Client Engagement and Support Establish and maintain professional relationships with assigned clients. Use trauma-informed, recovery-oriented, and person-centered approaches. Encourage client participation in their own treatment and recovery planning. Assist clients in identifying strengths and available support systems. Reinforce positive progress toward recovery and stability. Help clients develop problem-solving and self-advocacy skills. Encourage clients to utilize appropriate recovery and community resources. Maintain appropriate professional boundaries at all times. 1. Crisis Identification and Communication Recognize changes in client behavior, functioning, or circumstances that may require clinical attention. Immediately communicate significant concerns to the appropriate clinical or supervisory staff. Follow the organization's policies and procedures for emergencies, safety concerns, behavioral incidents, and crisis situations. Document significant events and interventions accurately and objectively. Do not perform clinical duties outside the employee's education, training, credentials, or scope of practice. 1. Community Outreach and Resource Development Develop and maintain knowledge of community resources available to individuals with substance use and behavioral health needs. Establish professional relationships with appropriate community agencies and service providers. Maintain an updated resource directory. Participate in community resource fairs, outreach activities, and agency meetings when assigned. Identify new resources that may benefit clients. Maintain professional relationships with referral partners. 1. Compliance and Confidentiality Maintain strict confidentiality of all client information. Follow applicable federal and state confidentiality requirements and organizational policies. Follow HIPAA requirements when applicable. Follow applicable substance use disorder confidentiality requirements, including 42 CFR Part 2, when applicable to the program. Follow organizational policies concerning release of information and authorization to disclose protected information. Never disclose confidential client information without appropriate authorization or a legally permitted basis. Follow all applicable licensing, accreditation, payer, contractual, and regulatory requirements. Complete required compliance and annual training. ADMINISTRATIVE RESPONSIBILITIES The Case Manager may be required to: Attend staff meetings. Attend treatment team meetings. Attend case conferences. Complete required reports. Maintain assigned caseload records. Track referrals and outcomes. Assist with audits and quality assurance activities. Participate in program improvement initiatives. Complete required training. Assist with client orientation when assigned. Assist with program activities as needed. Maintain organized and secure client files. Perform other administrative duties reasonably related to the position. MINIMUM QUALIFICATIONS The Case Manager should meet the education, experience, credentialing, and other requirements established by the applicable state regulations, licensing requirements, payer requirements, and organizational policies. • Generally preferred qualifications may include: Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Behavioral Health, Counseling, Sociology, or a related field. Experience working with individuals with substance use disorders, behavioral health conditions, homelessness, or other social-service needs. Experience providing case management, care coordination, community support, or social services. Knowledge of local community resources. Knowledge of substance use disorder treatment and recovery principles. Preferred experience: One or more years of experience in behavioral health, substance use disorder treatment, social services, case management, or a related field. CERTIFICATIONS Certification requirements shall be based on the applicable position, state regulations, payer requirements, and program requirements. Preferred credentials may include: Certified Peer Recovery Specialist (CPRS), where appropriate. Certified Alcohol and Drug Counselor (CADC), where applicable. Certified Case Manager (CCM), where applicable. Other behavioral health or case management credentials. Required certifications and credentials must remain current throughout employment. WORKING CONDITIONS The Case Manager may work in: Residential treatment settings. Outpatient treatment settings. Community-based settings. Office environments. Client homes or community locations when permitted. Other locations necessary to coordinate client services. The position may require frequent interaction with clients experiencing substance use, behavioral health, emotional, social, financial, housing, or other challenges. The employee must be able to maintain professionalism and appropriate boundaries in potentially stressful situations. PHYSICAL REQUIREMENTS The employee may be required to: Sit and stand for extended periods. Walk throughout the facility. Communicate verbally with clients and staff. Use a computer and telephone. Read and complete electronic and written documentation. Occasionally travel into the community for approved work-related activities.
