The Case Manager will assess patient needs, coordinate care, facilitate safe and appropriate discharge, support utilization management, and collaborate with patients, families, providers, and interdisciplinary teams. \n This position requires a minimum of 2 years of acute-care Case Management/Discharge Planning experience and prior travel experience. Behavioral health-only experience does not meet the requirement. \n Required Qualifications\n \n • 2 years of Acute Care Case Management/Discharge Planning experience required.\n, • Hospital experience required.\n, • Behavioral health experience alone does not qualify.\n, • Previous travel nursing experience required.\n, • Current RN license required.\n, • Pending California license may be accepted at submission but must be active by the start date.\n, • Compact RN license must be in hand at submission when applicable.\n, • BLS required.\n, • Strong experience with acute-care case management and discharge planning.\n, • Ability to independently arrange DME, SNF placement, and ambulance transportation.\n, • Strong utilization review and care-coordination experience.\n Core Responsibilities\n \n, • Assess patients' clinical, psychosocial, financial, and discharge needs.\n, • Perform comprehensive case management assessments.\n, • Coordinate care across the inpatient continuum.\n, • Develop and participate in individualized treatment and discharge plans.\n, • Facilitate safe, timely, and appropriate discharge planning.\n, • Conduct utilization review and evaluate admission/continued-stay criteria.\n, • Coordinate DME, SNF placement, ambulance transportation, and other post-acute services.\n, • Obtain and coordinate prior authorizations.\n, • Work with HMOs, IPAs, CMS requirements, and other healthcare organizations.\n, • Organize and participate in family meetings.\n, • Collaborate with physicians, nurses, social workers, physical therapists, and other interdisciplinary team members.\n, • Support patients and families in navigating healthcare resources.\n, • Document all case management activities accurately in the computerized medical record.\n, • Promote quality outcomes, patient satisfaction, and cost-effective care.\n Required Clinical Knowledge & Skills\n Candidates must demonstrate competency in: \n \n • Acute Hospital Case Management\n, • Care Coordination\n, • Discharge Planning\n, • Utilization Review\n, • Admission Criteria\n, • Needs Assessment\n, • Plan of Care Development\n, • Prior Authorizations\n, • DME Coordination\n, • SNF Placement\n, • Ambulance Transportation\n, • CMS – Centers for Medicare & Medicaid Services\n, • Department of Health requirements\n, • DRG knowledge\n, • Disease Management\n, • Emergency Department\n, • ICU\n, • Medical-Surgical\n, • Surgical Services\n, • The Joint Commission\n, • Core Measures\n, • National Patient Safety Goals\n Specialized Case Management Experience\n The successful candidate should be comfortable working with patients who may have complex social and medical needs, including knowledge of: \n \n • Mental health history and crisis situations\n, • Substance use and drug/alcohol treatment options\n, • Patients experiencing homelessness\n, • Advance directives\n, • POLST\n, • Protective services processes\n, • Adoption and foster-care considerations\n, • Complex discharge barriers\n, • Post-acute placement options\n \n The Case Manager must be able to independently coordinate appropriate resources and services. Patient Population\n The position may serve: \n \n • Neonates\n, • Adolescents\n, • Adults\n, • Geriatric patients\n \n Patient Ratio: Approximately 1:25 days; 1:25 weekends Daily Census: Approximately 25 Interdisciplinary Support: Physical Therapy and Social Services Documentation: Computerized documentation required Orientation: 40 hours Schedule\n \n • 5x8-hour day shifts\n, • 8:00 AM–4:30 PM\n, • Weekend coverage required based on department needs.\n, • Weekend rotation: approximately every other weekend.\n, • No scheduled on-call requirement indicated.\n, • Travelers may be expected to float between nearby sister facilities within the scope of their assignment.\n Travel Requirements\n \n, • First-time travelers are not accepted.\n, • Prior travel experience is required.\n, • Local radius: 50 miles.\n, • Local candidates may be accepted at a lower bill rate according to facility policy.\n, • Travelers must be prepared to work according to the contracted schedule and facility needs.\n, • Refusal to float to an appropriate nearby sister facility may result in termination and/or loss of guaranteed hours.\n RTO & Holiday Requirements\n \n, • Maximum preferred RTO: 7 days.\n, • RTO must be submitted with the initial application.\n, • Post-submission or post-offer RTO may not be considered.\n, • Requests exceeding 7 days may require client approval and potentially delay the offer.\n, • Traveler must commit to working 3 of 5 holidays:\n \n, • Thanksgiving\n, • Christmas Eve\n, • Christmas Day\n, • New Year's Eve\n, • New Year's Day\n \n Providence Facility Requirements\n \n, • Only one Providence facility may be submitted to at a time.\n, • Candidates must disclose whether they have previously worked for a Providence facility as:\n \n, • Permanent staff\n, • Travel staff\n, • Registry/per-diem\n \n, • If yes, the candidate must provide the facility location and dates worked.\n, • Former Providence staff may return after a 3-month separation.\n, • Facility does not accept Nursys or background-check hits.\n, • Two professional references are required during compliance, covering at least one year of experience within the last three years.
