Job Description Position Summary MaDorCARE is seeking a compassionate, dependable, and qualified Qualified Mental Health Professional–Community Services (QMHP-CS) to provide person-centered, recovery-oriented behavioral health services to children, adolescents, adults, and families experiencing mental health, emotional, behavioral, or co-occurring challenges. The QMHP-CS will work collaboratively with clients, families, treatment providers, schools, healthcare professionals, and community organizations to assess client needs, develop and implement individualized service plans, provide skills training and psychosocial rehabilitation, coordinate community resources, and support clients in achieving greater stability, independence, and improved quality of life. All services must be provided in accordance with the client’s treatment or recovery plan, MaDorCARE policies and procedures, applicable Texas Health and Human Services requirements, Medicaid standards, and the employee’s demonstrated competency and authorized scope of practice. Essential Duties and Responsibilities • Conduct behavioral health screenings, intake interviews, needs assessments, and other authorized assessments under appropriate clinical supervision., • Collaborate with clients, legally authorized representatives, families, and treatment team members to develop and update individualized recovery or treatment plans., • Provide community-based skills training, psychosocial rehabilitation, and supportive behavioral health interventions., • Teach clients practical coping, communication, emotional-regulation, problem-solving, social, self-care, and independent-living skills., • Assist clients in identifying personal strengths, treatment goals, natural supports, and barriers to recovery., • Provide case management, care coordination, referrals, resource linkage, and follow-up services., • Connect clients with medical, psychiatric, educational, housing, employment, transportation, financial, substance-use, and other community resources., • Monitor client progress and evaluate the effectiveness of services and interventions., • Provide crisis-prevention support, safety planning, de-escalation, and authorized crisis intervention in accordance with maDorCARE protocols., • Recognize and immediately report safety concerns, suspected abuse, neglect, exploitation, suicidal ideation, homicidal ideation, or other high-risk conditions., • Coordinate services with licensed clinicians, physicians, schools, hospitals, probation departments, child-welfare agencies, and other authorized providers., • Conduct services in clients’ homes, schools, offices, community locations, or through approved telehealth platforms., • Advocate for clients while promoting personal choice, dignity, independence, recovery, and community integration., • Maintain professional boundaries and protect client confidentiality., • Complete progress notes, assessments, treatment-plan updates, discharge documentation, incident reports, and other required records accurately and within established deadlines., • Ensure documentation clearly identifies the intervention provided, the client’s response, progress toward treatment goals, and plans for continued services., • Maintain compliance with Medicaid billing, service authorization, utilization-management, and documentation requirements., • Participate in clinical supervision, treatment-team meetings, case consultations, staff meetings, training, audits, and quality-improvement activities., • Maintain all required credentials, training, continuing education, background clearances, and demonstrated competencies., • Meet established productivity, caseload, documentation, service-quality, and client-engagement expectations., • Perform additional duties consistent with the position’s qualifications and authorized scope of practice. Minimum Qualifications • Bachelor’s degree from an accredited college or university in a qualifying human-services or behavioral-health field recognized under applicable Texas QMHP-CS requirements., • Ability to meet and maintain maDorCARE’s QMHP-CS credentialing and competency requirements., • Knowledge of mental health conditions, recovery principles, trauma-informed care, crisis prevention, and community-based behavioral health services., • Ability to work effectively with individuals from diverse cultural, socioeconomic, educational, and clinical backgrounds., • Strong assessment, documentation, communication, organization, and time-management skills., • Ability to work independently while following clinical supervision and organizational procedures., • Ability to maintain confidentiality and comply with HIPAA and other privacy requirements., • Ability to complete electronic health records and other documentation accurately and promptly., • Must successfully complete all required criminal-history, registry, and employment-background checks. Preferred Qualifications • Master’s degree in psychology, social work, counseling, marriage and family studies, behavioral science, or a related field., • One or more years of experience providing behavioral health, case management, psychosocial rehabilitation, crisis, or community-support services., • Experience working with individuals diagnosed with serious mental illness, severe emotional disturbance, substance-use disorders, or co-occurring conditions., • Experience providing Medicaid-funded mental health rehabilitative or targeted case-management services., • Familiarity with Texas Health and Human Services, Texas Resilience and Recovery, managed-care, or Medicaid documentation requirements., • Bilingual or multilingual communication skills., • Training in motivational interviewing, trauma-informed care, suicide prevention, crisis de-escalation, or evidence-based skills interventions. Knowledge, Skills, and Competencies The successful candidate should demonstrate competency in: • Person-centered and recovery-oriented service delivery, • Mental health screening and needs assessment, • Treatment and recovery planning, • Psychosocial rehabilitation and skills training, • Case management and care coordination, • Crisis prevention and de-escalation, • Community-resource identification, • Risk recognition and mandatory reporting, • Trauma-informed and culturally responsive care, • Clinical documentation and electronic health records, • Professional ethics, confidentiality, and boundaries, • Collaboration with multidisciplinary treatment teams, • Community-based caseload management, • Effective communication with clients and families Transportation and Scheduling Requirements • Valid driver’s license and acceptable driving record when community travel is required., • Reliable transportation and current automobile insurance when using a personal vehicle for work-related travel., • Ability to travel throughout the assigned service area., • Willingness to work flexible hours, which may include evenings, weekends, holidays, or participation in an on-call rotation, depending on program needs., • Ability to use a telephone, computer, electronic health record, and approved telehealth technology. Physical and Environmental Requirements The employee must be able to perform the essential duties of the position with or without reasonable accommodation. The position may require sitting, standing, walking, driving, entering client residences, carrying work materials, and responding appropriately in community environments with varying conditions. Clinical Supervision and Scope The QMHP-CS is not independently authorized to perform services reserved for licensed practitioners. The employee must work within their education, training, demonstrated competencies, assigned responsibilities, treatment plans, maDorCARE policies, and applicable supervision requirements.
