Job summaryWe are seeking a dedicated and professional Clinical Pharmacist to join our multidisciplinary team.This is a unique opportunity to contribute to a forward-thinking organisation within the Primary Care Network (PCN) framework. The PCN has already established excellent collaboration in delivering pharmacy provision to the many aspects of the DES within its locality. The post holder will help underpin and grow the team to move to working in line with the ever-evolving requirements of PCNs.Our current team is complemented by clinical pharmacists and pharmacy technicians working across all sites. This role is a replacement within the team to continue to grow on the support and accessibility of our PCN and its patient population.Main duties of the jobKey Responsibilities:1.Clinical Medicines ManagementProvide expertise in clinical medicines management, including medication reviews and ensuring safe prescribing practices, especially during care transitions.2.Medication ReviewsConduct remote or face-to-face structured medication reviews for patients, particularly those with polypharmacy, long-term conditions, or in care homes3.Support for PatientsOffer patient-facing support, addressing medication queries and optimizing medication use to improve adherence and outcomes4.Hospital Discharge ManagementReconcile and manage medicines post-hospital discharge, ensuring continuity of care and addressing unexplained changes5.Quality ImprovementLead clinical audits and contribute to quality improvement initiatives, including adherence to local and national prescribing guidelines.6.Service DevelopmentProvide pharmaceutical advice for creating and implementing new services with medicinal components7.Medicines SafetyReview and improve the safety of medicines associated with unplanned hospital admissions and high-risk groups8.Education and GuidanceProvide medication-related information to practice staff, healthcare teams, and patients, suggesting solutions and monitoring outcomes.9.Policy ImplementationMonitor compliance with local and national guidelines, assisting with practice formulary updates and adherence to NICE guidance.About usLAKESIDE HEALTHCARE is changing the face of primary care provision in England.
