As a Care Coordinator - Outpatient Services, you’ll be an important part of helping patients transition from hospitals and facilities to the care they need at home. You’ll work behind the scenes, and sometimes directly with patients - to make sure orders are accurate, insurance requirements are met, and equipment gets where it needs to go. What You’ll Do • Be the Connector: Bring together hospital teams, facility representatives, sales, delivery operations, patients, caregivers, and insurance providers to keep the discharge process moving smoothly., • Make Transitions Easier: Coordinate equipment and supply orders for patients leaving hospitals and facilities, helping create a seamless transition to home., • Be a Problem Solver: Review prescriptions, documentation, and insurance requirements to identify what’s needed to get an order moving forward., • Keep Things Moving: Create and enter orders into our electronic medical record and order management systems while keeping information accurate and up to date., • Work as a Team: Partner closely with our Hospital Liaisons, Sales Team, Delivery Operations, Billing, and other departments to make sure every patient receives the right equipment at the right time., • Connect with Patients: Reach out to patients and caregivers when needed to provide information, answer questions, and help them feel supported throughout the process., • Know the Details: Help ensure orders meet Medicare, Medicaid, and third-party insurance requirements and that all necessary documentation is in place., • Deliver EPIC Service: Bring empathy, passion, integrity, and commitment to every interaction with patients, referral sources, and teammates., • Jump In Where Needed: Healthcare is always changing, so you’ll have opportunities to take on new challenges and support the team wherever you can make an impact. Associate’s degree strongly preferred.
