Job Description The Performance Analyst serves as a data translator and domain advocate, bridging rigorous performance analytics with real-world operational context across CCN's external partner network. This role goes well beyond traditional reporting. It is a field-facing position and is responsible for developing performance frameworks, interpreting complex data through a clinical, social and operational lens, and proactively identifying opportunities for improvement across the region's health and social care landscape. The analyst engages directly with health systems, community-based organizations, County and state partners, bringing equal credibility to the numbers and to the care environment it describes, challenging assumptions in both directions: pushing partners toward what the numbers demand, and pushing back internally when the data tells a story leadership needs to hear. Essential Responsibilities Analytics & Performance Measurement • Leverage subject matter expertise in clinical care, behavioral health, and CBO operations to contextualize performance data, interpret findings through a clinical and operational lens, and translate them into actionable recommendations., • Collect, compile, and analyze data from multiple sources such as claims, utilization, workforce, financial, and population health sources using descriptive and inferential methods such as trend analysis, rate comparisons, confidence intervals and regression to surface patterns, gaps, and outliers across population and program data., • Design and maintain performance tracking frameworks, KPIs, and baseline measures across all CCN program areas. Define, track, and integrate performance metrics across current and future-state workflows, care coordination, workforce, and financial indicators to support planning, accountability, and strategic decision-making., • Develop scoring rubrics, feasibility and sustainability metrics, and outcome measures to support prioritization, logic model development, and program evaluation., • Build dashboards and reporting products that frame findings with narrative, clinical, and programmatic context, going beyond data display to deliver clear implications for decision-makers and governance bodies, with consistent methodology and data integrity across all sub-regions., • Support financial performance assessment including value-based payment opportunity analysis, revenue optimization tracking, and sustainability indicators., • Identify data quality gaps and interoperability requirements, and collaborate with the Business Intelligence team, external data sources, and sub-region partners to ensure consistent performance measurement across the region External Partner Engagement • Attend planning meetings and partner convenings and actively contribute to strategic discussions, and ensure metrics are aligned with evolving planning priorities., • Present performance findings directly to health systems, CBOs, DOH partners and other stakeholders as not just a hand off but as a facilitated conversation, helping partners understand their performance within the broader regional picture, building their capacity to engage with data meaningfully, and representing CCN's analytical perspective at convenings, planning tables, and governance structures. Advocacy & Organizational Intelligence • Proactively surface findings with strategic and/or operational implications, serve as an internal check on narrative drift by grounding planning discussions and leadership decisions in what the data actually supports, and develop recommendations that connect analysis to specific, actionable next steps, following through on whether those steps produce the intended results. MINIMUM REQUIREMENTS: • Bachelors degree in Business Analytics, Data Analytics, Data Science, Statistics, or a related field required; Master's degree preferred., • Minimum of 2 to 3 years of experience in health data analysis, research, or program evaluation, preferably in a health care or public health setting, especially handling Medicaid population data., • Experience with data analysis tools such as MS Excel, SQL, Power BI; familiarity with business intelligence platforms and data visualization tools preferred., • Strong analytical, and critical thinking skills with the ability to translate complex data into actionable findings., • Excellent written and verbal communication skills with the ability to prepare clear data summaries and reports for diverse audiences., • Ability to manage multiple data projects simultaneously and meet deadlines across deliverables.

Job Description Reporting to the Chief Business Intelligence Officer, the Performance Analytics Director provides strategic leadership across performance measurement and analytical support to sub-region partners. A core function of this role is serving as an embedded analytics resource for community-based organizations and other partners executing RHTP-funded projects, equipping them with the data tools, reporting frameworks, and analytical guidance needed to measure and communicate their impact. This role manages a team of Performance Analysts, each bringing subject matter expertise in clinical care, behavioral health, or community-based organization operations, and is responsible for building and sustaining a high-functioning, insight-driven performance function. The Performance Analytics Director oversees reporting, grounds governance bodies, planning teams, and sub-region partners in rigorous performance metrics while driving local engagement, ensuring active sub-region participation in analytics deliverables. This is a role that connects the rigor of performance data with the relationship management needed to turn insights into action on the ground. Essential Functions: • Lead, develop, and manage a team of Performance Analysts, fostering a culture of subject matter expertise, analytical rigor, initiative, and actionable insight. Supervise and support assigned staff and shared team members, providing day-to-day guidance, workload coordination and performance oversight to ensure effective delivery across all sub-region activities., • Provide strategic direction for the design and evolution of performance tracking frameworks, KPIs, and baseline reporting across all sub-regions, and collaborate with the Manager of Analytics on dashboards and reporting tools, ensuring outputs go beyond data display to provide narrative context, programmatic interpretation, and actionable implications for governance bodies and planning teams., • Serve as the primary analytics resource for sub-region partners, supporting data needs and analytical capacity for partner-executed projects; deploy and follow up on provider assessments, identify technical assistance needs, and track completion rates across partner organizations; and participate in workgroups and assessments to ensure sub-region context is reflected in measurement frameworks., • Design and administer surveys and data collection tools in support of community engagement activities led by the Planning team; facilitate access to county-level data sources and coordinate data access and data sharing agreements with public health, hospital, and social care partners; and coordinate workflow mapping, referral pathway design, and data sharing discussions with clinical, operational, and IT partners., • Proactively monitor performance data across sub-regions to identify variation from expected trajectories, conduct root cause analysis to diagnose drivers of underperformance, recommend targeted interventions to planning and partner teams, and validate improvement impact by tracking pre- and post-intervention data, supporting grant compliance reporting with evidence-based documentation., • Support workforce, financial, and implementation planning by coordinating local data collection and tracking analytic deliverable completion, reporting timelines, and data quality across sub-regions; and present performance findings and data insights at governance meetings, planning convenings, and partner forums, providing the analytical foundation for strategic discussions and ensuring performance data informs organizational decision-making. MINIMUM REQUIREMENTS: • Bachelor's degree in Data Analytics, Data Science, Public Health, Health Administration, Business Analytics, or a related field; Master’s degree preferred., • Minimum of 7-10 years of experience in health care, community health, or program coordination, preferably in a rural health or regional network setting., • Demonstrated experience translating complex data findings for non-technical audiences and building analytical capacity within partner or community-based organizations., • Strong organizational and project management skills with the ability to manage multiple deliverables and deadlines across sub-regions., • Experience coordinating data collection activities and working with diverse data sources including public health, hospital, and social care data., • Excellent written and verbal communication skills; ability to translate complex program requirements for diverse audiences., • Proficiency in Microsoft Office Suite (Word, MS Excel, PowerPoint, Teams); familiarity with project management and data visualization tools preferred., • Ability to travel within the assigned sub-region for partner and network meetings.
