SCN-Compliant Nutrition Services • Deliver individual and group-based nutrition counseling for Medicaid participants enrolled in the BWP., • Complete participant nutrition assessments to determine eligibility for MTMs and assist in customization of dietary plans., • Document all services in the SCN IT Platform, including service delivery, progress tracking, and participant feedback., • Collaborate with CHW and Program Manager to ensure that Social Care Plans align with medical nutrition therapy (MNT) goals. 1.
