Support Worker Job Purpose To provide person-centred care and support to individuals, helping them maintain their dignity, independence, wellbeing and quality of life while ensuring their safety and comfort. Key Responsibilities • Provide compassionate, person-centred support based on each individual's care plan., • Assist with daily living activities and encourage independence wherever possible., • Support with personal care, including washing, dressing, toileting and continence care, where required., • Assist with mobility, transfers and use of appropriate moving and handling equipment., • Support individuals with meals, drinks and maintaining good nutrition and hydration., • Provide companionship and emotional support., • Support individuals to attend appointments, activities and access the community where required., • Encourage participation in hobbies, social activities and meaningful activities., • Support individuals with dementia, learning disabilities, physical disabilities, mental health needs and other support requirements., • Observe and report changes in physical health, behaviour, mood or wellbeing., • Recognise and report safeguarding concerns, falls, pressure damage, accidents or other risks., • Follow care plans, risk assessments, medication procedures and safeguarding policies., • Maintain accurate and up-to-date care records and daily notes., • Maintain confidentiality and respect the individual's privacy and dignity., • Promote equality, choice, independence and person-centred care at all times., • Work effectively with colleagues, families and other professionals involved in the individual's care., • Maintain a safe, clean and comfortable environment., • Attend training and maintain required knowledge and skills.Person Specification, • Previous UK care or support work experience preferred., • Caring, patient, reliable and compassionate., • Good communication and listening skills., • Able to work independently and as part of a team., • Respectful and non-judgemental attitude., • Flexible and willing to work different shifts., • Right to Work in the UK., • Relevant care qualifications or training are desirable., • DBS check required.Working Hours Flexible shifts may include days, evenings, nights, weekends and bank holidays, depending on the client's requirements.
